Clinical Visit Information
Plan to arrive about half an hour before your clinic appointment to be sure that all of the necessary paper work has been completed. What to bring?
- Completed Neurosurgery Health Assessment Form
- A complete listing of medication
- MRI and CT scans (not just the reports, please)
- Medical Records (applicable to condition for which you are being seen, if done outside Vanderbilt)
- Insurance Card(s)
- Insurance Referral (if applicable)
- Insurance Co-payment (if applicable)
